The term personal support worker (PSW) is used almost exclusively within the Canadian province of Ontario. Why is that so?

Back in 1997 the Ministry of Health and the Ministry of Education and Training combined to create new program standards for workers that "provide personal care and support to people living at home and in long-term care facilities."

At the beginning of the 1997 school year a number of previous courses were combined into and governed under what is now today known as the "personal support worker training outcomes and module outlines standards."

Previously training for what are now considered personal support worker jobs feel under the umbrella of a number of courses. These courses consisted of home support worker levels I, II, and III training, along with a Health Care Aid course and finally Personal Attendant courses. Students enrolled in these courses in September and prior years were allowed to continue their studies, as the new standard applied to only new courses being offered from January 1st 1997 and on.

That takes care of the dry portion. So how come other provinces aren't jumping on the personal support guidelines bandwagon?

Since the personal support worker program is governed by the province of Ontario the degree has more value within Ontario than outside of it. Other provinces have different guidelines, training and rules. In Manitoba for example the tasks that an Ontario PSW carries out are handled by a "Health Care Aid". The system is less regulated in Manitoba. A student may acquire a health care aid certificate from certain employers while on the job at their facility. HPV is the abbreviation used for human papilloma virus. HPV causes the disease known as genital warts but it also causes cervical cancer. It is one of the most common sexually transmitted diseases in the world today.

There are many different types of HPV and not all types cause the wart like lesions to appear in the genital area. In fact most people who have this sexually transmitted disease do not know they have it.

Statistics suggest that up to 50% of sexually active adults will aquire HPV in their lifetime. If you were infected with the type of HPV that causes genital warts you may see the wart like lesions around the genital area, including on the shaft of the penis and the anal area. Women with genital warts may not see any lesions at all if they are confined to the vagina.

The strains of HPV that cause genital warts are classed as low risk viruses whereas the strains that cause cervical cancer are classed as a "high risk".
The link between cervical cancer and sexual activity was first suspected when the observation was made (1841) that nuns were never diagnosed wtih cervical cancer. At the same time it was noted that cervical cancer was more common in women who were prostitutes. A link between pappilloma viruses and cancer was found in research on rabbits but it was not until the human papilloma virus was identified that the link to HPV and cervical cancer was made. This occurred in the late 1970's.

Both the Canadian and American Cancer Society lists other risk factor for cervical cancer as long term exposure to female hormones such as those found in birth control pills, smoking, HIV infection, exposure to certain hormones such as diethylstilbestrol (DES), and genetic factors.

Cervical Cancer


The cervix is the opening of the uterus. When infected with the high risk human papilloma virus strains these cells can become cancerous. Women over 30 are at more risk to develop cervical cancer suggesting that it takes some time for the cancerous changes to occur after the HPV infection. Not all women who are infected with the human papilloma virus will go on to develop cervical cancer.

In the United States cervical cancer is more prevalent among the Hispanic and Black populations who also have a higher mortality from the disease. In Canada higher rates of cervical cancer are seen among the older (ages 40-59), immigrant, aboriginal, and lower socio-economic populations. In both countries it is thought that rates are higher among these populations because of lack of screening, which results in detection at a much later stage of the disease.

The number of deaths from cervical cancer in the last 50 years has decreased dramatically because of the ability to diagnose the disease early with the Pap test. This test bears the name of its originator George Papanikolaou. It is a simple test that requires a sample of cells from the cervix. The tissue is sent to a laboratory for examination under a microspope. Cancerous changes can be detected very early this way and early diagnosis improves success rates for treatment.

Treatment

When diagnosed early enough cervical cancer can be treated by removing the cancerous cells. This may range from removal of cells confined to the cervix to a complete hysterectomy depending on how advanced the cancer is and whether or not it has spread to lymphatic tissue.

Prevention

You can lower your risk of developing cervical cancer by limiting the number of sexual contacts and knowing your partner's HPV status. If you are sexually active and not in a monogamous relationship you should use a barrier method of protection with every sexual encounter and undergo regular screening as part of your routine health examination. It is recommended that sexually active women should have a gynecological exam with a Pap smear every three years to age 69.

HPV vaccine

Vaccines have recently come on the market that provide protection from two strains of high risk HPV. Health officials are recommending all young women be vaccinated before they beome sexually active. The vaccine is provided free of charge to girls in most provinces in Canada. Many states in the US are attempting to pass legislation requiring the vaccine but to date only 3 have been successful in getting legislation passed.

Orlando Weather In April

April weather in Orlando is comfortably warm and humid. With a normal high of 82 degrees Fahrenheit by the end of April, the daily temperature is warm, but not yet sweltering. Yes, if the temperature climbs toward the record of 95 it will be truly hot, but even then you will be able to tolerate the weather. You may want to consider one of the water parks if it does get that warm, however.

On the other end, freezing temperatures are really impossible. The record low for Orlando in April is 38 degrees Fahrenheit, but you are much more likely to encounter the normal low of 66. So, with a normal high of 82 and a normal low of 66, Orlando is a great place to defrost after a cold winter.

As for the chances of rain, it’s always possible to encounter rainy weather in Orlando. April isn’t in the rainy season, however, and there are typically just two in April in which over a half inch of rain falls, and 6 days in which .01 inches fall. Orlando businesses and theme parks always handle the rain well, since it happens fairly frequently, so you will probably, at worst, just have to wait a little while until the rain stops. Even if you are staying in a tent at Fort Wilderness in Disney the rain will only be a minor nuisance, if it affects you at all.

When you are packing, you may want to pack many more pairs of shorts than trousers, and more short-sleeved shirts than long-sleeved shirts. But do take a light jacket, just in case, and a rain poncho, along with sunscreen.

And do bring a bathing suit and beach towel, and swim toys for the kids. Swimming will be very enjoyable on almost any day in April in Orlando. The water parks will be a lot of fun as well. The Orlando water parks include Sea World’s Aquatica, Wet ‘N Wild, and Disney’s Blizzard Beach and Typhoon Lagoon. All of these parks are incredible fun for kids and adults.

You may also choose to enjoy other, drier outdoor activities. If you like to walk you may even find that you prefer the early morning over later in the day, as the mornings are cooler, and the same goes for running. Golfers will find the weather to be comfortable most of the time, maybe even hot in the late afternoon. You may want to schedule an earlier tee time, although it’s not as necessary in April as it will be in May or later.

There aren’t any obvious places to visit near Orlando that will have significantly different weather. You would have to head west to the Gulf of Mexico or east to the Atlantic ocean in order to enjoy any coolness near the beach. There’s no significantly elevated land nearby that would offer any different climate, either. So be prepared for warm and humid weather, and a small chance of rain, and come see one of the greatest tourist spots in the world.

Creating the Ultimate Vision for Ultimate Success

I've been thinking a lot lately about the importance of having a clear, compelling vision to get you past all of life's challenges and obstacles.

In fact, it prompted me to do a little research about some of our society's most successful people. For example, did you know that:

Einstein's teachers told his parents he would never amount to anything.
Thomas Edison went broke four times.
Michael Jordan was cut from his high school basketball team.
A newspaper editor fired Walt Disney for having no good ideas.
Steven Spielberg was put into a class for students with learning disabilities.
Colonel Sanders talked to 1005 people before he could find one person willing to try his recipe. That makes his closing ratio 1005 to one - undeniably the worst I've ever seen.

What's inspiring to me about these highly successful people isn't just their eventual accomplishments. It's how they were able to overcome their early failures and go on to achieve greatness.

How did they succeed where so many others fail? Because they each had a grand and sincere vision of what they wanted to accomplish. With a grand vision, there are no such things as permanent failures. There are only temporary setbacks.

Ultimate success comes from having a strong vision. To me, a strong vision is all about setting clear goals. So how can you find your ultimate vision? Try asking yourself some of the following questions:

What do you want to have?
Brainstorm wildly with this one, and don't hold back. List everything you've ever dreamed of having. A new BMW. A trip around the world. Or maybe just a couple of beach houses in Maui. Whatever it is you desire, write it here.

What do you want to experience?
Perhaps a new culture? Parenthood? Or simply the pleasure of never again having to call anyone else abossa?

What are you willing to give up?
Achieving what you want is about making and managing choices, so ask yourself what you're willing to give up in order to achieve your goals.
If you want to grow your business by 50% this year, for example, maybe you have to give up your C-rated customers. Maybe you have to give up an hour of sleep a night so you can come into the office and make your sales calls earlier. Or maybe you have to give up working out seven days a week to stay at the office later.

What's your ideal environment?
What is the ideal environment you like to work and play in? What do you want to do with your time outside of work, or when you go home? Maybe it's riding motorcycles, learning to cook or writing a screenplay. Maybe it's having children, or spending time with your family. Or it might just be as simple as going to the beach, reading trashy novels or watching more movies.
If you're staring outside at a blizzard and feel tears begin to well up in your eyes, maybe your ideal place to work and play is in Florida or the Barbados. On the other hand, if the first snowflake sends you running for your skis, you're probably already in the right place.

What are you grateful for?
Record where you are today, and compare it to where you were two, three, five or even 10 years ago. Note the differences, celebrate your successes - and resolve to change those things in your life you aren't happy with.

How long do you expect to live - and when do you want to retire?
What do you plan to do after you retire? What do you want to achieve? And if you end up living ten years longer than you expected to, what will you do with your time then?

For that matter, why wait? Is there anything on your retirement wish list you can start doing right now?

When answering these questions, make sure you spend some uninterrupted thinking time. It could be on an airplane or train, on weekends, in front of the fire or while you're on vacation. Ideally, you should choose someplace that's both comfortable and stimulating. But wherever you do your thinking, don't give the development of your vision short shrift. After all, as U.S. Senator Conrad Burns reminds us:

aIn life you are given two ends, one to think with and the other to sit on. Your success in life depends on which end you use the most. Heads you win, tails you lose.a

Once you have the answers to these questions, surround yourself with people who will help you achieve your vision. My father always said - Colleen, if you want to be rich, study from people who are already rich. Similarly, you will more easily achieve your vision if you associate with people who are already living it. Of course, things change over time, so be sure to revisit your answers at least once a year to make sure your vision stays up to date with your lifestyle and what you've accomplished. It's okay for your vision to be dynamic, just so long as what doesn't change is your commitment to it.

Regardless of any short-term changes that occur in your life, however, you may be surprised to find that your grand vision hardly changes at all. If you answer the questions above as honestly as you can, your vision today will likely be very similar to the vision you create five years from now, because they will both come from your core hopes, dreams and values.

Ekalaka, Montana: The Days Of '85

Ekalaka became an official town in 1885 and was the business center for the area; the two closest settlements were Miles City and Camp Crook. Cattle ranching and buffalo hunting were demanding occupations where horses were necessary. Races matching local driving and riding horses provided a popular form of entertainment in early-day Ekalaka as there were large numbers of swift running steeds in everyday use.

A half-mile circular race track was laid out on Ekalaka"s west side and a big race meet took place, July 7, 1897. The well attended meet was a financial success and 2-day race meets were afterward held every September.

A group of businessmen and race horse owners formed an organization in 1912 to raise money to purchase the Ekalaka Race Track. Eighteen acres of land were acquired from William Sweeney for $1,000. The new organization, Ekalaka Fair and Racing Association, held their first fair and race meet in late summer attracting 1,800 people.

A new wooden grandstand (28x100 feet) was constructed on the south side of the race track in 1915. The well-built structure remained in use until 1974 when it was torn down to make room for a new grandstand.

Bronc riding and rodeos increased in popularity in the early 1920"s. Two bronc saddling chutes were built on the north side of the race track directly in front of the grandstand in 1925 and horses bucked on the track. Bronc riding, horse races and the fair headlined Ekalaka"s late summer celebrations through the 1920"s and into the 1930"s.

The Ekalaka Fair and Racing Association sold the race track grounds to Carter County in 1930 and a new high school was built on the grounds" east side in 1938.

New bronc chutes were put in place across the race track east of the grandstand. Horses bucked and calves were roped in a large arena north of the race track. The bronc chutes were later relocated on the arena"s northwest corner.

The Town of Ekalaka reached the half-century mark in 1935. The Chamber of Commerce sponsored a gigantic celebration to commemorate Ekalaka"s first 50 years""the celebration was named, "The Days of "85" and the name has since symbolized Ekalaka"s summer fair and rodeo. Ekalaka"s Golden Jubilee featured horse races, Indian dances, street dances, boxing matches, buffalo, a carnival, a western parade with scores of floats, horses and riders, while top rodeo cowboys attempted to ride Tooke Brothers bucking horses. For years afterward, old-timers talked about the great time they had at the Golden Jubilee.

Days of "85 celebrations were not held from 1950 through 1953. Feek Tooke leased the fairgrounds in 1954 and revived the Days of "85 Fair and Rodeo which has continued to be an annual Ekalaka summertime event for 54 years.

Throngs of people converged on Ekalaka for the Diamond Jubilee in early August, 1960. A cloudburst Saturday morning failed to dampen everyone"s enthusiasm and the colorful parade kicked off a full weekend of festivities. Rodeo cowboys from Florida to Canada competed for prize money in a muddy arena. A carnival provided fun for young and old; street dances, barbeques and class reunions were also enjoyed by old friends.

Ekalaka"s Cen 00004000 tennial Days of "85 in August, 1985, filled the historic town with people. There was an all-school reunion at the high school, street dances, a 2-performance rodeo, and a truly spectacular parade that lasted two hours. It was estimated that 6,000 people were in Ekalaka for Sunday morning"s parade.

The rodeo arena was moved to Ekalaka"s southwest side to allow construction of a new high school in 1980, but livestock barns and a new Event Center remain on original fairgrounds land. Everything is in place for Ekalaka"s 125th birthday in 2010.

What Should my First Credit Card Entail?

There are several different credit cards on the market. Some have a lot of benefits, yet some don’t really give back anything in return except a high interest rate. Getting your first credit card is a big time in a young adult’s life. You must make sure you know how to use them wisely, you don’t get yourself into debt, and that you follow the rules of a credit card.

If you’ve just turned 18 years old, and you’re looking for your first credit card, let me give you some advice.

• You want to pay each balance off on your credit card in a full amount. Meaning, if you put on $75 this month, you will want to pay off $75 all at once. If you don’t pay it off in full and let’s say you only paid off $30, you will get interest on that last $45. This can be very pricey over time and that $45 will no longer be $45, but more instead next month.

• Never go over your credit limit, if you do, you will most likely get charged a fee.

• Don’t ever miss a payment. Do all that you can to make sure you know when to pay your bills! Whether you set a reminder on your cell phone or you mark it on the calendar. Make sure you pay it on time so that you don’t hurt your credit score.

• You also want to make sure you get a credit card with good benefits. If you’re going to stick to one card, you might as well get treated well and rewarded.

This is what your card should entail:

• Find a card that has a low number of fees or better yet, one that doesn’t have any fees!

• Stick with a company that has good customer service. If ever you have a problem, you want them to treat you well.

• Look for a low interest rate in your credit card. Anything over 20% is getting too high. Look for the lowest one possible.

• Also, look to get rewarded for each dollar you spend. Some credit cards will give you one point per dollar spent. Once you’ve earned enough points, you can then turn them into gift cards, prizes, donations, or even money towards your bill sometimes.

• If you don’t get a credit card that gives you points per dollar spent, make sure you get a card that gives you cash back. Some cards will give you up to 5% cash back on each purchase you make. This is a great way to save money on everything!

Looking for a credit card can be tough the first time around. We’re not too sure what to look for and we don’t know what everything means. Unfortunately when we’re naive about credit cards, the companies can take advantage of us! Antiques and Collectibles in Mexico: San Miguel de Allende, Mexico City (Lagunilla), Puebla (Los Sapos) and Oaxaca

The Sunday open air antiques stalls at Lagunilla in Mexico City, the expansive roadside collectibles shops just north of San Miguel de Allende, the stores and weekend marketplace at Los Sapos in Puebla, and good old fashioned picking in the state of Oaxaca. Each provides a fruitful avenue for acquiring antiques and collectibles in central and southern Mexico. Of course there are many more, but over the past two decades this transplanted Canadian has found success pounding the pavement (often in the case of Oaxaca barely passable dirt roads and pathways) in these four venues.

The Selection of Antiques and Collectibles Available in Central & Southern Mexico

While the selection and quantity of antiques available in central and southern Mexico is impressive, those searching for depression and other collectible glass might be in for a surprise. There is very little glass from American, Canadian and European factories available in Mexico, relative to what one finds in Canada and the US. And when one does come across quality antique glass, in most cases it's expensive. However, hand-blown glass (vidrio soplado) has been manufactured in Mexico since the 16th century, though a different quality than the glass one encounters back home. Mexican glass is relatively common and priced to sell, usually in excellent condition in terms of original hand-painted designs and without chips or cracks.

One comes across a fair bit of military memorabilia including weapons, vintage books and coins, tiles and other ceramic pieces, advertising signs for products and cinema, as well as other smalls. Naturally, religious artifacts are prevalent, including retablos, ex votos, cherubs and crosses.

Iron has also been forged in Mexico since the 16th century, generally holding up well with time. In fact ironworkers in modern Mexico, at least in the southern half of the country, are arguably the best of all the building trades in terms of workmanship. Locks and keys, railings, gates, frames, in addition to tools and weapons and a plethora of other iron products, are encountered without difficulty in Lagunilla, Los Sapos, San Miguel de Allende, and even in the few antique stores in Oaxaca.

Collectible stone pieces are available in virtually all shops and markets, in particular grinding stones (referred to as metates with manos - the hand piece) used for mashing corn, and mortar and pestle sets (known as molcajetes) for pulverizing predominantly spices, herbs and chiles. One sometimes stumbles upon hand-hewn limestone cornices off of convents and government buildings.

Both rectangular and dome - topped wooden chests are widespread. The painted or stripped baAl (pine blanket box for Americans and Canadians) is often found with its original four - legged base. Doors off of administrative buildings and ex - haciendas are massive in terms of height, width and thickness, frequently found with original hardware in tact. Tables, wagon wheels and implements round out the other main wooden collectibles one can find throughout this part of Mexico.

Select Locales for Finding Antiques and Collectibles in Central & Southern Mexico

The Sunday open air Mexico City antiques and collectibles market known as Lagunilla extends for several blocks, and is accessible by walking from any of the hotels close to the zAcalo (central square), and of course by taxi. There are a few antique stores in the area as well, although the vendors with stalls constitute the main attraction at Lagunilla.

Many travel books caution about safety and security at Lagunilla, and some dealers warn about being in the area approaching dusk. However antique hunters should be fine, provided normal precautions are taken: do not venture off to what would appear to be a "seedy" area; do not flash large wads of cash; keep cameras and purses in front and close to the body; and yes, it would be imprudent to wander around the area as night approaches.

The quaint quarters known as Los Sapos, in downtown Puebla about four blocks from its zAcalo, are also a haven for collectors and dealers. The weekend market is admittedly small, especially for those accustomed to the Christie Classic Antique Show at Dundas, Ontario, the expansive sales at Brimfield, Massachusetts, and similar large, outdoor antiques and collectibles markets in Canada and the US. But one can find gems at Los Sapos, both by scrounging through the Saturday and Sunday stalls (not all the same vendors attend both days), and to a lesser extent in the shops within three or so blocks of the open - air marketplace. Lamps and chandeliers stand out, especially in the stores, at prices hard to resist.

Highway 51 leading out of San Miguel de Allende en route to Dolores Hidalgo is a fruitful route for finding antiques, especially larger pieces. Prices are surprisingly reasonable, given that many of the ex-patriots living in San Miguel de Allende are of significant means. It's curious that prices tend to be exorbitant San Miguel proper, yet accessible only a few miles away in the several shops and sprawling outdoor antique yards flecking both sides of the highway. Get out of the city, be it with a rental car or by hiring a driver, and stop at every outlet.

While Oaxaca does have one extremely large antique store (on Calle Abasolo) with a particularly impressive selection of jewelry (and almost everything else), for its size Oaxaca is a wasteland for collectors and dealers, and prices are steep relative to what one finds elsewhere in central and southern Mexico. You have to go to the rural areas.

Oaxaca is noted for its craft villages, market towns and colonial churches in the countryside. But these towns and villages have been pretty well picked over since the travel boom which began in the 1960s. Accordingly, it's imperative to venture beyond the usual tourist stops. Virtually all of the antiques and collectibles enumerated above can be found in Oaxaca's hinterland, in addition to the occasional early craft item (i.e. fanciful wooden alebrijes, ceramics, textiles).

The central valleys of Oaxaca are purportedly where the chango mezcalero originated. Chango mezcalero is a baked clay receptacle painted as a monkey, used for holding and serving mezcal, the alcoholic beverage derived from the agave plant. The same $20 whimsically painted ceramic bottle produced beginning around the 1930s, fetches $500 or more on a couple of Mexican antiques websites. But like everything else, they're getting harder to come by.

Cautionary Notes for Antiques Aficionados Traveling in Central & Southern Mexico

Mention of pre-Hispanic artifacts is conspicuously absent from the foregoing. There are two reasons:

  • The law prohibits buying and selling archaeological pieces, and of course their export. One periodically hears of even Mexicans winding up in jail or subject to stiff fines as a result of trading in pre-Hispanic pieces.
  • Reproductions represented as originals are big business, even at the pre-Hispanic ruin sites, another reason to stay clean of archaeological pieces. If it's represented as a repro, then go ahead. Query how vendors at Lagunilla can flog pieces they represent as legitimate pre-Hispanic artifacts, out in the open, if they are indeed originals. Campesinos sometimes approach foreigners stating they have just come across pieces while plowing. While certainly it happens, and there continues to be artifacts being unearthed all the time, these farmers have access to reproductions as well.

Other antique and vintage collectibles which are being reproduced and are sometimes represented as old, include advertising signs, metal cantina trays, ex votos, papier machA "puta" dolls, ceremonial masks and ironwork. Of course well - made reproductions are often quite attractive and are suitable as home decor, but unless certain, don't pay prices which correspond to the value of true vintage collectibles.

In some cases contemporary well - worn implements may appear to be antique, but are not; nor is there an attempt to misrepresent. Take for example, metates. Some are pre-Hispanic, while others could be only 30 - 50 years old, since some Mexican women today still grind corn over a large flat river rock. After decades of use it appears no different than a metate which was worked 1,500 years ago.

Another class of collectible that may or may not be antique, yet without attempt to misrepresent, is galvanized metal containers in a variety of shapes and sizes, for making tamales (tamaleras), and for carrying milk, water and other liquids.

Wood is at times difficult to bring into the US and Canada. The laws in Australia are even stricter. Pine, copal and other soft woods are susceptible to insect infestation, akin to termites. The problem is known as polilla. Tell-tale signs are tiny holes in the wood, or if it's been sitting in one place for a while, a white powder can be found alongside the piece. If in a shop, look around the base before picking up anything wood.

When buying contemporary collectible alebrijes, and other wooden products, ask what precautions have been taken to prevent polilla infestation. In San MartAn Tilcajete, Oaxaca, the high end workshop of Jacobo Angeles and MarAa Mendoza uses both soaking in a gasoline / insecticide mixture, and leaving in a special oven for a number of hours, so as to enable the workshop to guarantee its work.

Finally, resist the temptation to awake before dawn to get to the markets before anyone else. In most cases antiques and collectibles dealers with stalls at the outdoor marketplaces do not arrive and set up at the crack of dawn, like they do at markets like Christie or Brimfield. If you arrive at 8 a.m. (before virtually all other pickers and collectors) you'll be there while the vendors are setting up, and if you arrive at 10 a.m. there will still be stalls being arranged. Having provided this advice, guess who nevertheless awakens at 7 a.m., and gets to the antiques markets as soon as possible thereafter, invariably first succumbing to relaxing at a sidewalk stand for an early breakfast of hot chocolate and tamales, yes, before heading off to the market, thereafter of course, scrounging for Mexican antiques and collectibles throughout the better part of a day?
